Appliance Repair Invoice Template

Built for repair techs billing a no-spin washer, a refrigerator that waits a week for a control board, and a home-warranty call โ€” model and serial on every ticket, the diagnostic fee credited in plain sight, and the warranty written down.

Most appliance repair disputes are about the first $89. The customer was told there was a diagnostic fee, and was told โ€” or thinks they were told โ€” that it comes off the repair. Then the invoice arrives with the diagnostic on one line, the repair on another, and nothing that says whether one was credited against the other. Or the repair was declined, and the customer is surprised to pay anything at all for a washer that still doesn't spin. One clear line settles both cases.

Appliance repair also has paperwork other service trades don't. The model and serial number decide which part fits, whether the manufacturer's warranty still covers it, and whether there is a recall. Parts are often special-ordered, so many jobs take two trips and a deposit. And a lot of calls come through home warranty companies or landlords, where someone other than the person at the door pays most of the bill. This template puts the appliance, the diagnosis, the parts, the trips and the warranty on separate lines, so each of those questions has an answer on paper.

How appliance repair work is priced

Most independent appliance techs charge a diagnostic or service call fee to come out and find the fault โ€” commonly a flat amount that includes travel โ€” and credit it toward the repair if the customer goes ahead. Repairs are then priced either as flat-rate jobs from a price book (a dryer belt, a washer pump, a dishwasher drain pump each at a set price, parts included or separate) or as parts plus labor by the hour. Parts that aren't on the truck are special-ordered; shops commonly take a parts deposit before ordering, then bill a return trip or include it in the repair price. Sealed-system refrigerator and freezer work (compressor, leaks, recharge) must be done by an EPA Section 608 certified technician and is priced higher, with refrigerant often listed separately. When the repair isn't worth it, the customer pays the diagnostic and gets a written recommendation. Home warranty and manufacturer warranty jobs are billed to the warranty company under its authorization number, with the customer paying only the service fee and anything not covered. Most residential jobs are paid on completion; landlords and property managers usually pay on net 15 or net 30 against a work order.

What belongs on a appliance repair invoice

These are the lines this trade actually bills, the unit each is measured in, and what each one prevents. Every line below is optional โ€” use the ones that match the job.

Line itemBilled asWhy it belongs there
Diagnostic โ€” Whirlpool top-load washer, no spin; model WTW5000DW1, serial C81204567flat, per visitName the appliance, the complaint, and the model and serial number. The model decides the part, and the serial decides whether the manufacturer's warranty or a recall applies.
Diagnostic fee credited to approved repaircredit (negative line)If you credit the diagnostic, show it as its own minus line. 'Applied to repair' in a note is easy to miss; a line with a minus sign is not.
Part โ€” drive motor coupler, OEM part #W10820048per partGive the part number and say whether it's OEM or aftermarket. It lets the customer check the price, and it lets you match a warranty claim to the part later.
Labor โ€” replace coupler, test spin and drain cyclesflat-rate job or per hourSay what you did and what you tested. A tested cycle on the invoice is your answer if the customer calls back about a different fault.
Special-order part deposit received โ€” main control board, ordered 10/9 (enter as negative credit)credit (negative line)When a part has to be ordered, take a deposit and show it as a credit on the final invoice. It protects you from a non-returnable part the customer no longer wants.
Return trip โ€” install ordered part, 10/14flat, per tripEither bill the second trip or say it's included. Leaving it unsaid is how a $40 line turns into a phone call.
Sealed-system repair โ€” leak at evaporator, brazed, evacuated, recharged with 4.5 oz R-600a (EPA 608 certified tech)flat-rate job + refrigerantSealed-system work is the most expensive line on any appliance invoice. Name the leak, the refrigerant and the amount, and that the tech is certified.
Not economical to repair โ€” dishwasher, failed tub and pump; replacement recommended, customer declined repairrecord, $0When the repair isn't worth it, write that down next to the diagnostic. It explains why the customer paid for a visit and still has a broken dishwasher.
Labor warranty โ€” 90 days on labor; parts per manufacturer's warrantyrecord, $0Write the warranty on the invoice. It saves a call-back argument, and it makes a call-back for a different fault easy to tell apart from a warranty claim.
Home warranty claim #_, authorization #_ โ€” service fee collected from homeowner; balance billed to warranty companyper claimWarranty-company jobs have two payers. Put the claim and authorization numbers on the invoice and keep anything not covered on a separate, customer-approved line.

Appliance Repair invoicing questions

Should the diagnostic fee be credited toward the repair?

Most independent appliance techs do credit it when the customer approves the repair, and it's a strong reason for customers to say yes. Whatever your policy, say it before the visit, and then show it on the invoice: the diagnostic as one line and, if credited, a separate minus line reading 'Diagnostic fee credited to approved repair.' If the customer declines the repair, the diagnostic stands on its own and the invoice should say the repair was declined.

What should an appliance repair invoice include?

Your business details and the customer's, an invoice number and date, the service address, and then the appliance itself: brand, type, model number and serial number. After that, the diagnosis, the diagnostic fee and whether it was credited, each part with its part number, the labor, any return trip, any deposit received as a credit, the warranty on the work, and the total with payment terms. For home warranty jobs, add the claim and authorization numbers.

How do I invoice a repair that needs a special-order part?

On the first visit, bill the diagnostic and write an estimate for the repair. If the customer approves, take a parts deposit before ordering and give them a receipt. When the part arrives, do the repair and write one final invoice: the repair, the part, the return trip (or 'included'), the diagnostic credit if you give one, and the deposit as a minus line, so the balance due is clear.

How do I bill a home warranty company for an appliance repair?

Follow the warranty company's process: get an authorization number before the work, collect only the service fee the warranty company tells you to collect from the homeowner, and bill the rest to the warranty company with the claim and authorization numbers on the invoice. If the homeowner wants work the warranty doesn't cover, get their approval and put it on its own line, billed to them.

Home Repair & Service Calls: templates that bill together

Service-call trades bill a diagnostic or trip fee, then parts and labor as separate lines โ€” and the dispute is almost always whether the first fee was credited against the repair.
